Job Description



The RN Case Manager/Discharge Coordinator collaborates with the patient/family and members of the
health care team to ensure a safe and effective transition through the
continuum of care.  Duties will include arrangement of post-acute care
needs to include DME, home health, SNF or LTAC placement.  Interacts with
payer sources to obtain authorization of post-acute services, and documents all
interactions with patients/families per departmental and regulatory
standards.  



 Actively participate in the Treatment Team meetings.
 Participate in the assessment, treatment planning, and discharge planning
process.
 Serve as the primary spokesperson for the treatment team with the patient,
family, case workers, outpatient therapists and other professionals.

 Assume an active role in discharge planning by identifying barriers to
discharge during initial assessment and ongoing interactions with the
patient and family.
 Formulate discharge plan in coordination with the treatment team, patient,
family, and outside agencies as appropriate.
 Establish and maintain cooperative relationships with community agencies
and other resources.

 Interact with patients, family members, treatment team, and external
entities professionally and respectfully.
 Ensure the treatment environment supports the program’s mission to
provide exemplary treatment and service.
 Inform patients and family members of treatment scope and limitations,
offer treatment options when possible and build upon the strengths and
interests of the patient.
 Inform patients and family when treatment or overall status notably
changes.
 Provide information related to patient and team members’ role in the
treatment process.

 Communicate issues which require follow-up to the Director of Social
Services.
 Work collaboratively with all treatment team members to create a treatment
environment that supports the facility’s mission to provide exemplary
treatment and customer service.
 Act to preserve patient and family dignity and safety.

 Documentation for medical records and reports is timely, legible, thorough,
and in required format.
 Document as required including assessments, interventions, treatment plan,
discharge planning, and incident reports.
 Create, Update and revise treatment plan as assigned and per policy.

QUALIFICATIONS
EDUCATION: Graduate of an accredited nursing program.
CERTIFICATIONS/LICENSES: Current State Licensure. CPI/CPR Certified per policy.
EXPERIENCE: Two years as Registered Nurse with one year acute unit based psychiatric experience preferred. Minimum of one year of psychiatric discharge planning experience preferred.
